Multiple copies of one unique

How can you get multiple copies of for example Mr.Iron in a single run? I have sen that in several screenshots of long runs.

Rise of Cthulhu Supporter
Mr Iron was not unique in the past so in older versions you could have multiple of him on the field. Normally you can not get more than one of a unique or legendary except for bugs.
